ELECTROCARDIOGRAM


Here are the most basic things I can talk about this diagnostic tool.

The Sheet:

5 mm = 1 large box
1 mV = 10 mm (2 large boxes)
1 large box = 0.5 mV
1 small box = 0.1 mV = 1mm
25 mm/s = normal paper speed
25 mm = 1 sec = 5 large boxes = 25 small boxes
1 large box = 0.2 sec = 5 mm
1 small box = 0.04 sec


Normal durations of some of the waves & segments:

β€’ P-wave = <0.12 sec (< 3 small boxes)
β € β—‹ If prolonged: Second peak in lead II = P mitrale (enlargement of LA)
β€’ PR interval = 0.12-0.2 sec (3-5 small boxes)
β € β—‹ AV conduction time
β € β—‹ If >0.2 sec = first degree AV block
β € β—‹ If <0.12 sec = pre-excitation syndromes (accessory pathways)
β€’ QRS = <0.1 sec (1.5 small box)
β € β—‹ Depolarization of ventricles
β € β—‹ Prolonged = complete bundle branch block

Limb Leads:

R arm - red
L arm - yellow
R leg - black
L leg - green

Precordial Leads:

V1 (R), V2 (L) = sternal borders @ 4th ICS = above septum
V3 - bet V2 and V4
V4 = 5th ICS MCL
V5 (anterior axillary line) = same IC plane with V4
V6 (midaxillary line) = same IC plane with V4 & V5
V7 = posterior axillary line
V8 = scapular line

Territories:

V1-V4 anteroseptal
V1, V2 - Septal wall
V3, V4 - Anterior wall
V5, V6 - Lateral wall
1, AVL - High lateral wall
2, 3, AVF - Inferior wall
RV - more anterior wall

My personal order of assessment when I read an ECG strip:

1. Rhythm (also check upright P waves in leads II, III, AVF)
2. Heart rate (300/large boxes between 2 R's, or if (+) arrhythmia: # of QRS complexes in 6 mins * 10)
3. P-waves
4. PR interval
5. QRS axis
6. QRS duration
7. Rotation
8. QRS amplitude
9. QRS Infarction signs
10. ST segment
11. QT duration
12. T wave changes

About the Sidris Creature

Sidrises are artificial hearts designed by the Science and Research Center in Ark City for patients requiring a heart transplant. They take less than 30 seconds to hatch upon demand and function just as any normal heart would but with an extra touch. Sidrises are conscious of their own health and cooperate with Arkian doctors to improve the quality of life of patients with heart disease.

In order to minimize future complications, Sidrises will alert their bearer of check-ups through a specific warning beat pattern. Similarly, they will also noticeably react if dietary restrictions are violated or exercise levels are not met. That's a whole lot of love from a heart!
